45-49 Year Health Assessment at Green Square Health

This assessment is designed for patients between the ages of 45 and 49 who may benefit from a comprehensive health assessment to identify any risk factors for chronic disease early, and act on them to prevent complications in the future.

Why is the 45–49-year-old health assessment important?

A health assessment in the 45-49 age range is available for patients who are interested in proactively managing their health and make necessary lifestyle changes to maintain their well-being. This assessment aims to identify any potential health risks and provide guidance for a healthier lifestyle.

It aims to pick up on silent risks like blood pressure, blood sugar, lifestyle factors that may have crept in, and risks that rise without our awareness.

Eligibility for the 45-49 Year Health Assessment

Are you wondering if you are eligible for the 45-49 Year Health Assessment? Here’s what you need to know:

Medicare Eligibility Criteria:
  • If you’re Medicare Card holder aged 45-49 and have risk factors and a family history of disease, you may be eligible for the subsidised health assessment.
Private Eligibility Criteria:
  • If you don’t have a Medicare card, or fall just outside of this age group, you can have a health assessment performed privately with a smaller time based rebate.
Identification of Risk Factors:
  • To be eligible for the health check, your GP needs to identify at least one risk factor. Risk factors are based on:
    • Family history of chronic disease.
    • Blood pressure, cholesterol, or blood sugar levels.
    • Weight and diet.
    • History of smoking.
    • Alcohol intake.
    • Exercise habits.
Medicare Rebate:
  • If you’re eligible for the health assessment, Medicare will pay a one-off rebate to support your health and well-being with no out of pocket cost to you.

The Green Square Health Process

At Green Square Health, our process for the 45-49 Year Health Assessment is:

  1. Nurse Evaluation (45 minutes): Patients will initially meet with our experienced nurse for a comprehensive evaluation. This step typically takes about 45 minutes.
  2. GP Review: Following the nurse evaluation, patients will have a thorough review with their General Practitioner (GP). We recommend that you undergo this assessment with your regular GP who is familiar with your medical history and can provide personalised care. During this review, the GP will discuss findings from the nurse evaluation, conduct additional assessments if necessary, and provide personalised recommendations for your health and well-being.

Want to get a head start?

Here are some things to think of before coming in for your assessment;

  • What kind of sleep are you getting?
  • What is your diet like? What is typical in a day for you?
  • Try find out a bit more about your family history. First degree relatives matter most (parents, siblings), then extended relatives for certain serious conditions
  • What medications have been prescribed that you are taking now, or that you have stopped taking. Always good to try remember when it was started, and for what purpose you take them
  • Do you take any supplements or vitamins?
  • What are your goals for your health? (seems a silly question – but specific goals like avoiding diabetes, going for daily walks when I’m 85, making sure I can travel).
